We respect your right to privacy and keep all your health information confidential and secure. It is important that the NHS keeps accurate and up-to-date records about your health and treatment so that those treating you can give you the best possible care.
This information may be used for management and audit purposes. However, it is usually only available to, and used by, those involved in your care. You have the right to know what information we hold about you. If you would like to see your records please contact the Practice Manager.
Fair processing notice
The Lindum Medical Practice asks for personal information so that we can ensure that you receive the information, advice, treatment, care and support that is right for you. Information about you, the services you receive and your family background may be recorded either on paper or computer files as part of providing you with health and social care services.
We keep records of:
- Your name, address and date of birth
- Why you or someone on your behalf contacted us and when
- The type of services you were offered or received
- Relevant family information
Anyone who receives patient identifiable information from us is also under a legal duty to keep it confidential. We take the sensitivity of personal information very seriously in the Practice. Staff and contractors are required to respect their duty of confidentiality codes of conduct and through their contracts of employment.
Sharing of information is strictly governed by information protocols and the NHS Code of Practice for confidentiality. These are produced in accordance with legislation and good practice and are designed to protect your rights. We are also registered under the Data Protection Act. We abide by all statutory requirements regarding confidentiality, whilst following the appropriate guidelines for information security. All our staff are required to work within these guidelines.
From July 2014 NHS England and The Lindum Medical Practice will be reviewing patient information in order to identify those patients most likely to be admitted to hospital. This is a national scheme called “Avoiding Unplanned Admissions” where the intention is to pro-actively manage the care of those patients identified as being at most risk and pre-empt an unplanned admission to hospital. The information gathered is for the practice use only and only your GP and practice staff will be involved, however should you wish to opt out or if you have any questions, please leave your contact details at reception.

Enhanced data sharing
Enhanced Data Sharing is different from the Care Data Sharing on which you will all have received a leaflet of explanation from the Government.
Enhanced Data Sharing is the sharing of Data between different Health organisations and Public health organisations such as Community Nurses or Podiatry
As a Patient you have two options with regards to allowing the sharing of Data held at your GP surgery.
You can allow the Practice to share out your Data to allow other services to see your record or you can refuse this to happen. This is called Sharing OUT
Also you can allow the GP practice to see other data from other services you have used such as the Community Nursing Service;The Out of Hours service or the Walk in Centre or again you can refuse this to happen. This is called Sharing iN.
